Job Summary:
We are seeking a highly skilled Case Worker/Case Manager to join our team and will play a vital role in providing essential support services to individuals in need. Will be conduct comprehensive assessments guide families through the application process and connect them with appropriate mental health services. Dedication and expertise will contribute to positive outcomes and improve the lives of those we serve.
Key Responsibilities:

Conduct facetoface CNAS assessments with youth and families.

Manage incoming referrals ensuring initial contact within 48 business hours.

Guide families through the application and documentation process for CMHW eligibility.

Build and maintain relationships with mental health service providers for seamless transitions.

Participate in training and development sessions to stay updated on best practices.

Track document and report program data via the Tobi database system.

Travel within assigned counties for assessments and followups.
Education and Requirements:

Bachelors degree in Social Work Psychology Counseling Nursing or a related field with 2 years of clinical experience; OR

Masters degree in Social Work Psychology Counseling Nursing or a related field with 2 years of clinical experience.

Valid Indiana drivers license and current vehicle insurance.

Clear Department of Child Services (DCS) background checks.

Strong interpersonal skills for engaging with diverse families and providers.
