Job Description
- Conduct facetoface CNAS assessments with youth and families.
- Manage incoming referrals ensuring initial contact within 48 business hours.
- Guide families through the application and documentation process for CMHW eligibility.
- Build and maintain relationships with mental health service providers for seamless transitions.
- Participate in training and development sessions to stay updated on best practices.
- Track document and report program data via the Tobi database system.
- Travel within assigned counties for assessments and followups.
- Bachelors degree in Social Work Psychology Counseling Nursing or a related field with 2 years of clinical experience; OR
- Masters degree in Social Work Psychology Counseling Nursing or a related field with 2 years of clinical experience.
- Valid Indiana drivers license and current vehicle insurance.
- Clear Department of Child Services (DCS) background checks.
- Exceptional time management to meet 15businessday referral processing deadlines.
- Strong interpersonal skills for engaging with diverse families and providers.
- Proficiency in documenting and navigating database systems like Tobi.
- Commitment to upholding HIPAA and other confidentiality standards.
