Job Description

Job Title: Social Worker

Location: Tucson AZ

Start Date: Feb 2025

Duration: 1 Year or Full time (options for 26 weeks contract)
Shift Schedule: 12 hrs (48hrs/week)

Qualifications

Education: Graduate from a nationally accredited school with an MSW.

Experience: 2year experience as Social worker

License: Any state license will work LCSW

Responsibilities:

1.Perform Biopsychosocial Assessments.

2.Assesses patient and plan appropriately to assist with social determinants of health

3.Suicide Risk Mental Health and Crisis Assessment and intervention.

4.Document safety plan and other plans of care and care given in patients medical record.

5. Assist with discharge planning for veterans who need resources when going home such as home health consults.

6. Reviews all information in the electronic medical records pertinent for social work services.
