Job Description
Position Name: Physical Therapist Home Health Immediate Opportunity!
Location: Paducah KY 42003
Start Date: ASAP
Employment Type: Fulltime Travel Contract Temporary
Contract Length: 13 weeksAccepting Local and Travel Candidates
Pay Rate: $2360 to $2400 Weekly Gross pay (Nontaxable travel/housing and M&I stipend available for qualified/eligible travelers)
Unit: Home Health
Shift: 8 Hours; Days 8a 5p
Schedule: 40 hours / week guaranteed 5 shifts per week MonFri No weekend or call
Position Requirements:
- Current Physical Therapist experience in a acute care or rehab setting : 2 years Required
- Recent/Current Home Health Nursing experience: Highly preferred
- Home Care Home Base (HCHB) charting experience Preferred
- Must have valid Drivers License and Personal Auto Insurance
- Must be Covid Vaccinated or provide medical or religious exemption
Education License and Certification Requirements:
- Bachelor of Science degree in Physical Therapy or Similar Required
- Current State License as Physical Therapist or Compact if applicable Required
- BLS Certification from AHA or American Red Cross Required
Job Description:
- Provide Physical Therapy services to patients/clients at their homes.
- Consult with the physician and an interdisciplinary team in the development of a plan of treatment
- Utilize skills in patient/client evaluation and treatment professional consultations to achieve maximum patient/client independence in accordance with the individuals functional level and potential
- Plans and prepares treatment plan based on evaluation of patient data
- Provide inhome PT treatment programs to assist patients in restoring function relieving pain and preventing disability
- Allow patients and their families to maintain dignity and independence while receiving safe effective rehabilitation in the security of their home
- Drive to patient homes in the assigned territories on daily basis.
- Territories to cover: Livingston Ballard Carlisle and Marshall counties
