Job Description

Job Summary This position provides care to assigned residents in a caring safe and efficient manner and is responsible to perform the following according to location standards procedures and individualized resident care plans. Essential Duties and REsponsibilities Completes the assigned portions of the MDS for Nursing. Signs the MDS forms to signify completion of the process. Maintains residents selfesteem privacy and confidentiality of personal information. Assists to maintain safe pleasant orderly resident living areas. Completes nursing portion of MDS and reviews for completion and validity of entire process. Queries other members for accuracy of MDS; and provides final quality check prior to MDS transmission. Receives and shares information. Observes and reports residents conditions. Communicates with physicians using center/campus systems. Directs and assists with coworker resident family/significant other education. Provides support and recognition to coworkers related to completion of the RAI / MDS process. Uses the EMR accurately. Basic Responsibilities: This position will be held accountable for complying with all related laws regulations company policies and procedures pertaining to his or her position and for fulfilling his or her obligations under the Societys Corporate Compliance Program. Duties responsibilities and activities may change at any time with or without notice. Education Experience Licensure Certification: Graduate of an accredited school of nursing. RN nursing licensure for state of practice is required. Must be certified through AANAC as a Resident Assessment Coordinator (RACCT) within 6 month of hire. Skills/Abilities: Ability to communicate and comprehend. Knowledge: View/Print Job Description Page 1 of 2 7/20/2018 Knowledge of the RAI/ MDS process; knowledge of basic nursing skills. Physical/Mental Requirements: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job the employee will operate/activate/use/prepare/inspect/place/detect/position objects tools or controls. The employee will frequently communicate/express oneself/exchange/observe/assess/detect information relative to this position. The employee is required to maintain a stationary position/move/position self in response to job tasks. Must be able to effectively communicate in English both orally and in writing.
