Job Description

Position Summary:

We are seeking a dedicated Nurse Practitioner to join our team. Under the supervision of the Medical Director you will collaborate with physicians and the interdisciplinary team to provide comprehensive assessments care planning primary care and ongoing medical education to our valued participants.

Essential Duties and Responsibilities:

  • Conduct inperson assessments including initial semiannual and unscheduled evaluations to ensure continuous medical coverage for participants.

  • Collaborate with physicians and the interdisciplinary team to develop and implement individualized care plans.

  • Participate in discharge planning from acute or longterm care settings ensuring a smooth transition for participants.

  • Provide regular visits to participants in nursing homes based on their individual needs.

  • Work closely with WelbeHealth physicians communitybased PACE physicians and the interdisciplinary team to facilitate effective care coordination.

  • Under the guidance of the Medical Director review outcomes and utilization data to identify areas for improvement and establish best practices.

  • Participate in rotating night and weekend oncall coverage.

Qualifications:

  • Graduation from an accredited School of Nursing.

  • Current certification as a Nurse Practitioner with Prescriptive Authority.

  • Valid RN State License.

  • Minimum of two years of experience working with the senior population.

  • Uptodate knowledge of chronic care and geriatric issues.

  • Reliable transportation.

Salary Range:

$145000 $174478 per year plus bonus and equity.
