Job Description
Mobile Health Team Inc. a staffing agency has an opening for a permanent fulltime Social Worker Emergency Room position. We are looking for someone with a passion for people.
Location: Wilmington DE 19801
Start Date: 11/25/2024
Shift: 1 opening is 3p1130p 1 opening is 11p730a Both Full Time
Salary: $62000.00 to $85000.00
Successful candidates may be eligible for a $15000 signon bonus to be paid within the first year of a twoyear commitment.

THE CARE MANAGEMENT MODEL:
Our Care Management Triad Team Model is a collaboration between the following:
- RN Case Manager manages patient care drives patient progression and establishes a discharge plan.
- Social Worker resolves psychosocial barriers and supports discharge needs.
- Utilization Management review patient status for appropriateness and anticipated payer coverage.
EXPERIENCE/EDUCATION/QUALIFICATION
Experience:
- Must have at least 2 yrs of experience in acute care.
- Acute Care Social Work experience preferred.
- The maternalchild position requires experience working with neonates children and families
Education:
- Masters degree in Social Work (MSW) or Master of Social Services degree (MSS) from an accredited graduate school is required before the hire date.
Licenses Certifications and/or Registration:
- Licensed CSW or MSW as required by the state of Delaware before the hire date.
- CCM is preferred within 18 months of eligibility.
