Job Summary
The Licensed Professional Counselor (LPC) will serve as an integral part of the mental health care team collaborating with healthcare providers to deliver therapy using evidencebased brief interventions. The role involves helping individuals with behavior modification coping skills and other therapeutic interventions aimed at improving mental and physical health and overall wellbeing in a primary care environment. The LPC will work closely with primary care providers and staff to establish and manage care plans. LPC LMSW or LMHC credentials are accepted.

Responsibilities:
Conduct patient screenings and symptom assessments to determine the most appropriate therapeutic interventions.
Collaborate with providers and primary care teams to counsel patients individually or with family members and caregivers ensuring all members of the care team are informed.
Maintain strict confidentiality of patient records.
Guide patients families and caregivers in developing coping strategies to overcome emotional stress trauma and other barriers.
Assist patients in navigating referrals for different levels of care as needed.
Coordinate mental health care with an integrated care team including physicians providers care managers and support staff.
Complete detailed documentation in the companys EMR system based on patient assessments and interventions.
Evaluate patients mental and physical conditions developing treatment plans based on clinical knowledge and experience.
Act as a client advocate to coordinate services and resolve emergency issues during crisis situations.
Serve as the behavioral health expert for the care team providing guidance on behavioral health topics and trends.
Required Qualifications:
Licensed as LPC LMSW CADC or LMHC.
Masters degree in psychology counseling or a related field.
Valid state credentials and licensing.
Experience working with a diverse patient population including adolescents and geriatric patients is preferred.
Prior experience in a primary care setting is a plus.
Strong understanding of human behavior personality learning and motivation as well as behavioral health assessment and treatment.
Proficiency in using electronic health records (EHR) for documentation.
Knowledge of diagnostic treatment and rehabilitation methods for physical and mental health conditions.
Familiarity with group dynamics societal influences and cultural factors impacting health.
Strong customer service and personal service skills.
Physical Requirements:
Ability to lift up to 40 pounds.
