Job Description
Hilltop Village provides short and longterm nursing and rehabilitation services to the senior population in our community. Hilltop Village is fully committed to providing our residents and employees with a true family homelike setting where all employees look forward to going to work to provide the highest quality health care services possible. Develop and implement the Physical Therapy plan of care for facility residents and shortterm patients. Assist as needed with patient screenings for the need for Physical Therapy. Complete timely PT documentation in compliance with regulatory licensing and payor requirements. Provide and document patient family and caregiver training regarding each PT patient s specific plan of care and recommendations for follow up. Current Texas licensure as a Physical Therapist required. Healthcare setting: Nursing home Medical specialties: Ability to commute/relocate: License/Certification: Work Location: In person
