Job Summary
We are seeking a compassionate and skilled Registered Nurse for our Home Health services. The ideal candidate will provide highquality nursing care to patients in their home environment ensuring their comfort and safety. This role requires strong clinical skills the ability to handle diverse patient needs and a commitment to patient education and support.

Duties

• Monitor and assess the condition of patients in their home setting.
• Collaborate with physicians and other healthcare professionals to develop and implement comprehensive care plans.
• Administer medications perform interventions and manage patient care as necessary.
• Educate patients and their families about treatment processes medications and followup care.
• Maintain accurate and thorough patient records using electronic health record systems such as Epic or Meditech.
• Ensure compliance with infection control protocols and patient safety standards.
• Participate in quality improvement initiatives aimed at enhancing patient outcomes and workflow efficiency.

Experience

• Minimum 2 years of experience in Home Health or similar communitybased setting.
• Experience with chronic disease management preferred.

Requirements

• Active CA State RN license.
• BLS certification required; ACLS certification preferred.
