Job Description

Join our Family Support Services Team!

The Moore Center s Family Support team is seeking a fulltime InHome Supports Family Manager to coordinate client services assist in the oversight of direct support professionals and develop client budgets. This includes evaluating client needs prioritizing the level of service coordination determining appropriate and costeffective services tracking expenses and documenting outcomes.

Responsibilities include:

  • Demonstrate knowledge of NH Regs. (HeM) and ensure compliance with regulations
  • Develop and implement the individualized service budget with the HeM524 regulations
  • Work with HR and Family Managers to recruit and supervise staff; implement HR best practices
  • Process vouchers and invoices for companion sitters and services being covered through the waivered services within the individual s budget.
  • Conduct and review assessments of client needs
  • Develop a Service Agreement including goals objectives methodology and timelines based on assessments
  • Ensure service delivery meets client needs in an appropriate setting with positive outcomes
  • Coordinate RFP process to create a new or different service provider as necessary
  • Negotiate service contracts with vendors that demonstrate quality services
  • Demonstrate an understanding of client budgets
  • Resource community organizations to reduce costs and increase client supports
  • Demonstrate competency in the use of the accountable care concept

Qualifications:

  • Bachelor s degree preferred; consideration may be given to those with an Associate s degree and a minimum of 2 years of work experience in human services
  • 12 years of experience working in human services preferred
