Job Description
Medical Social Worker (MSW) Napa CA (Adult Day Care Community/Outpatient Community) Contract Dates: ASAP Shift: 5x8hr Days (MonFri; Weekends not required) Years of Experience REQ: 1 year of prior social work / clinical experience in a healthcare setting REQUIRED; Charge experience REQUIRED (Preferred settings: adult day care case management home healthcare LTC/nursing homes rehab) FirstTimers Accepted: Yes Weekend REQ: No Certs REQ: BLS Is oncall REQ No Will clinician float within scope to meet facility needs No Open to accommodating block schedule No Locals Accepted: Yes; Considered local within 50 mile radius will accept locals at the same rate Pending License Accepted: No; License NOT REQUIRED for this role RTO Restrictions: Confirm with hiring manager Guaranteed Hours: Facility may call off traveler UP TO 1 shift per pay period (every 2 weeks) SPECIAL REQUIREMENTS Candidate is expected to have reliable transportation and no additional scheduling restrictions. All travel pairs will be required to have their own transportation. The Medical Social Worker conducts psychosocial evaluations and formulates treatment plans and goals to assist with resolving problems and supports participants in meeting their biopsychosocial needs. Makes referrals to appropriate community resources. Confers with other disciplines. Participates in internal meetings. Completes documentation in a timely manner. Assists in community outreach efforts in conjunction with the Program Director and Counseling/Social Work Manager.
