Job Description
Responsibilities
1. Supports organizations mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with coworkers
2. Assesses diagnoses and develops appropriate plan of care to treat homebound patients
3. Monitors the patients condition and responsiveness to the treatment provided
4. Acts as an agency representative with patients and their families as well as with other health agencies groups and organizations
5. Participates in organizational wide QAPI program and attends agency sponsored inservice programs
6. Adheres to agency infection control and safety policies
7. Adheres to the organizations policy in regards to absenteeism and appearance
Qualifications
1. Masters degree in Nursing required from an NLN accredited program
2. Licensure as an Advanced Practice Nurse (APN) in NJ with prescriptive authority; maintain a CDS and DEA
3. National certification as an Adult or Geriatric Nurse Practitioner
4. 2 years of APN experience and/or significant experience as a registered nurse
5. Excellent communication organizational and interpersonal skills
6. Strong computer skills
