As a Nutritionist, you will play a vital role in providing dietetics, nutritional counseling, and education aimed at improving health and wellness.
Key Responsibilities:
Nutritional Assessment: Utilize relevant tools and protocols to assess patients’ nutritional needs, creating tailored profiles to guide their wellness journey.
Customized Plans: Develop and personalize dietary, fitness, and wellness plans for patients enrolled in our chronic care program, focusing on individualized care.
Patient Education and Motivation: Educate, counsel, and motivate patients to implement necessary lifestyle changes for effective diabetes management.
Holistic Approach: Create treatment plans that respect patient preferences and facilitate realistic goal achievement, adopting a comprehensive approach to care.
Patient Engagement: Maintain a high-touch connection with patients through various communication channels (chat, video, audio calls) to support their progress and ensure goal achievement.
Community Outreach: Provide diabetes education to patients, their families, and the broader community, promoting awareness and understanding of nutritional management.
Data Utilization: Demonstrate a strong understanding of data-driven nutrition and lifestyle management, leveraging devices such as Continuous Glucose Monitors (CGMs) and glucometers to inform patient care.
Customer Satisfaction: Strive to achieve high customer ratings by delivering exceptional service and support.
