Required Qualifications:
Associate degree or higher
1 years of experience in ANY of the following:
Electronic Medical Record (EMR)
Other EMR
TruChart EHR
Valid Licensed Practical Nurse (LPN) in the state of Pennsylvania (PA)
ANY of the following valid licenses/certifications:
American Heart Association Advanced Cardiac Life Support (AHA ACLS)
American Heart Association Basic Life Support (AHA BLS)
Please provide a candidate LPN License Number.
Territory/Driving Expectation: Lower half of Dauphin County
Top Experiences: Previous Home Health/Hospice experience any type of acute care LTC care
Top Misses (experiences we dont want): pretty much open to experience at this point for decent candidates
Schedule: MondayFriday 8am5pm one weekend every 48 weeks Saturday & Sunday 8am5pm

Our client is seeking a compassionate Home Health LPN to join their extraordinary care team in Harrisburg PA. The ideal candidate will have an Associate degree LPN license in the state of work and two years of LPN experience. Responsibilities include collaborating to provide patient care plans supporting patients in the local area and demonstrating proficiency with technology tools.
Key Responsibilities:
Collaborate to provide management and delivery of patient care plans
Support patients in Harrisburg PA and surrounding areas
Meet critical deadlines for charting
Selfmanage time and schedule efficiently
Demonstrate experience with tablets mobile phones and EMR software
