Job Description
Join our healthcare client a leading rehabilitation facility providing postacute care. We are seeking a licensed Physical Therapist to deliver personalized care enhance patient outcomes and contribute to a collaborative healthcare environment.
Key Responsibilities:
- Develop and implement treatment plans with physician approval.
- Treat patients according to prescribed plans and monitor progress.
- Supervise Physical Therapy Assistants in accordance with state regulations.
- Assist in training Restorative Aides and support discharge planning.
- Document evaluations progress notes and recertifications per regulations.
- Provide training for families and caregivers to ensure smooth discharge.
- Attend care conferences participate in staff training and recommend medical equipment.
- Foster positive relationships with patients families and staff.
Qualifications:
- Valid Physical Therapist License
