Job Description
Blanchard Valley Health System in NW Ohio is looking for an experienced NP/PA in their Caughman Health Center.
Job Description:
- Three years outpatient family medicine experience REQUIRED will not consider anyone with less
- Adults 18 years
- Flexible days and hours
- No hospital call light phone call 1:3
- Fulltime average 25 patients per day
- Cerner EMR hospitalemployment
- Hospitalist group admits all patients
- Join full support staff
- Seeking longterm commitment to ensure continuity of care
- Caughman Health Center provides both family practice and pediatric care to area residents regardless of ability to pay. For patient convenience there is an on site laboratory pharmacy assistance program social workers and a care navigator. All fees are based on a sliding scale and most insurances are accepted including Ohio Medicaid and selfpay. Financial assistance available for those who qualify. Fees are based on ability to pay.
- Not accepting new graduate candidates experience required. CURRENT 3 years of the last 5 years in Outpatient Family Medicine experience is required.
- Not accepting Locums candidates
- Not accepting visa candidates

Requirement description :
ALL OF THE FOLLOWING ARE REQUIRED AT NAME CLEAR UNLESS NOTED OTHERWISE.
- Board Certified NP/PA REQUIRED
- OH license (or willing to obtain) REQUIRED
- DEA in hand/able to transfer REQUIRED
- CSR/CDS in hand/able to transfer REQUIRED
- Current 3 years of the last 5 years in Outpatient Family Medicine experience REQUIRED (NO EXCEPTIONS)
- Date of agencys last communication with provider (put answer in Additional Submission Details) REQUIRED
- Why is candidate interested in BVHS (put answer in Additional Submission Details) REQUIRED
- Does the provider require visa support J1 or H1B (note in Additional Submission Details field) REQUIRED
