Shift times are 8a8p
Needing 23 days of locum coverage per week in December
3 days a week for Jan/Feb
Standard Credentialing is 6090 days
All ages seen
EMR EPIC
Cold and Flu Symptoms: Aches Cough Fever Nasal Congestion
Sinus and Allergy Symptoms: Itchy Eyes Nasal Congestion Nasal Discharge Facial Pressure and Pain Cough
Conjunctivitis (Pink Eye)
Diarrhea
Ear Infections
Eye Irritation
Headaches
Sexually Transmitted Diseases (STD)
Strep Throat
Insect Bites
Rashes: Poison Ivy Ringworm Shingles Scabies Hives
Minor Burns
Minor Ses Cuts or Bruises
Minor Injuries: Sprains Muscle Strains
Nausea and Vomiting
Joint Sprains
Urinary Tract Infections (UTI)
NO OT
Orientation 4 hrs paid
Holiday rate is 1.4x
Rates to submit: Orientation Hourly Holiday Conversion
Expenses:
Hotel Not to exceed BR00/night
Flight reimbursed coach ticket if distance is > 150 miles
Mileage reimbursed if distance is < 150 miles
Client is not responsible for meals laundry dry cleaning telephone tv or entertainment
Requirement description :
- Board Certified/Board Eligible Strong preference for BC or a set date for testing for taking the boards. REQUIRED
- IL state license REQUIRED no exceptions
- Covid Vaccination REQUIRED
- Rates to submit: Orientation Hourly Holiday Conversion (holiday is 1.4x)
- DEA Verbal Confirmation Required at Name Clear

- Missouri
ADDITIONAL LICENSE REQUIREMENTS :
Weekend Requirements : Yes expectation is to rotate with other providers
