Please note time off requests must be disclosed with initial submission to be considered for approval. Time off requested after the acceptance date will not be approved. 13 weeks assignment at least one years experience current required in outpatient physical therapy. Must be a team player flexible and accustomed to fast paced environment. Must have successfully completed at least one 13week travel assignment. When submitting candidates include at least two references (one must be from current manager) a resume and completed skills checklist/competency assessment.DUTIES: Evaluates and treats patients regarding the application of a wide variety of therapeutic interventions for rehabilitation of physical function and developmental disorders. *Must demonstrate knowledge of the principles of growth and development over the life span of patients served *Oversees and ensures a comprehensive rehabilitation program designed to meet the physical psychosocial and immediate environmental wellbeing of the patient *Promotes patient/caregiver selfdetermination by optimizing outside resource utilization to achieve the highest level of independence. *Coordinates and communicates with various disciplines or services involved in patient care. *Coordinates efforts toward discharge goals established at initial evaluation and during episode of care. *Is a patient advocate.CERTIFICATIONS: Must have a PT License. BLS/BCLS certifications required. BLS must be completed through American Heart Association American Health and Safety or American Red Cross.SHIFTS: Starts between; Hours Flexible (5 x 8s; 4 x 10s etc.) Shifts may fluctuate to fulfill department staffing needs. No Weekends Holidays or Call. Travelers are first to float within facility and to alternate locations within 20 miles of permanent assigned location. Traveling pairs are not guaranteed same schedules or same shifts. If traveler is required to float (unlikely) they will float to support local OP Rehab facilities. ORIENTATION: 1st week of the contract not to exceed 40 total hours and includes EPIC training. Scrubs required and not provided by facility
