Home Health LPN

Position: Home Health LPN
Location: Grand Junction CO
Duration: 13 weeks
Pay scale: $1800/wk
Shift: Monday Friday 8 am5 pm

Responsibilities:

• Provides nursing care to client consistent with physician orders established care plan client choice and regulations.

• Accountable for observation of the client and documentation regarding observations/care given.

• Updates clients field chart and medication sheets when changes occur as a result of communications with other members of the team and/or care provided.

• Coordinates clients care with physician family members of the interdisciplinary team and other involved agencies.

• Provides teaching and instruction to clients/caregivers to promote physical and emotional health.

• Assures client rights are protected and that all clients/caregivers are made aware of the clients rights and responsibilities.

• BLS Required
  
• Compact or LPN License Required
