Job Description
What you will be doing:
Conduct comprehensive patient essments and physical examinations.
Diagnose and manage acute and chronic health conditions.
Develop and implement individualized patient care plans.
Prescribe medications and other treatments as necessary.
Provide preventive care including immunizations and health screenings.
Educate patients and families on health maintenance and disease prevention.
Collaborate with physicians and other healthcare professionals to ensure coordinated and comprehensive care.
Maintain accurate and uptodate patient records.
Participate in community outreach and health promotion activities.
Stay current with medical advancements and best practices.
Experience you will need:
Masters or Doctoral degree in Nursing from an accredited program.
Certification as a Family Nurse Practitioner (FNP) or equivalent.
Valid APRN license to practice in New Hampshire.
Strong clinical ss and knowledge of primary care.
Excellent communication and interpersonal ss.
Commitment to providing highquality patientcentered care.
Ability to work collaboratively in a teamoriented environment.
Experience in community health or underserved populations is a plus.
