Job Title: Registered Nurse Case Manager
Location: Eureka CA 95501
Duration: 3 Months
Job Type: Contract
Work Type: Onsite
Job Description:
- We are seeking a compassionate and skilled Registered Nurse to join our Case Management team at our Magnet status hospital.
- This position is also available at other locations: Santa Rosa Orange Tarzana and Eureka offering you flexibility and opportunity across multiple settings.
Responsibilities:
- Conduct admission concurrent and retrospective reviews.
- Ensure accurate care determinations and obtain necessary preauthorizations.
- Follow Medicare guidelines for utilization review.
- Facilitate communication among care team members.
- Coordinate postacute care services and family meetings.
- Act as a patient advocate throughout the care process.
Qualifications:
- Graduate of an accredited nursing program with an active RN license.
- Minimum of 1 year of Case Management experience.
- Current AHA BLS certification.
- Proficiency in Microsoft Office/Teams and Epic (preferred).
- Additional Information:
- Weekend Requirement: Required to work every other weekend.
- COVID19 Vaccine: Required with exemptions allowed.
