Job Description
- Managing the behavioral and mental health care of patients.
- Review patients medical records to ensure that their plan of care is being followed and that patients medical management needs are being met.
- Working collaboratively with behavioral coordinators PMDs and psychiatrists to resolve any gaps inpatients treatment plans.
- Your responsibilities will include chart audits and reviews medical charting maintaining a patient registry and attending care team meetings.
Requirements
- Valid LPN license in NY
- Basic healthcare background experience
