Position Title: Licensed Practical/Vocational Nurse (LPN/LVN)

Job Description;
Under the supervision of a registered nurse the Licensed Vocational/Practical Nurse assumes responsibility and accountability for the application of the nursing process and delivery of patient care. The LPN/LVN consistently performs according to nursing standards and is responsible for managing patient care while assisting others in the management of patient care.
SUPERVISED BY AND RESPONSIBLE TO:
The nurse manager or designee of the unit assigned at the hospital or facility and the Medical branch manager or designee.
POSITION REQUIREMENTS:
- Current practical/vocational nurse license.
- Full command verbal and written of the English language.
- Current physical and PPD as per facility requirements.
- Prefer a minimum of one year of facility staffing experience within the last three years.
- Current BLS card.
- Successful completion of screening tests with a score of 70% or greater.
RESPONSIBILITIES:
(Under the direct supervision of a registered nurse)
- Utilize the nursing process to assess plan implement and evaluate patient care.
- Perform focused interviews to identify specific patient needs.
- Assess signs and symptoms indicating physiological changes in the patient s condition.
- Collect analyze and interpret data from healthcare team members and document actual and/or potential nursing diagnoses.
- Document the patient s plan of care using identified nursing diagnoses expected patient outcomes and selected nursing interventions.
- Identify and prioritize actions based on patient care requirements.
- Monitor consistency between the patient plan of care and actions of medical staff.
- Perform interventions according to identified priorities plan of care and hospital policies and patient care outcome standards.
- Provide a safe comfortable therapeutic environment for patients.
- Initiate and evaluate patient and/or family education.
- Revise the plan of care based on evaluation changes in medical plans and effectiveness/ineffectiveness of interventions.
- Document the patients response to nursing and other interventions.
- Use clinical judgment in evaluation activities to meet patient care needs on the assigned unit/floor including establishing priorities.
- Monitor the safety and effectiveness of the environment and equipment.
- Participate in activities that facilitate good interpersonal communication with the chosen facility.
- Support professional practice by adhering to:
- Nurse Practice Act
- Standards of Nursing Practice
- Standards of Patient Care
- JCAHO standards
- State and local laws
- Patient s Bill of Rights
ESSENTIAL JOB FUNCTIONS:
Employee is required to stand walk bend/stoop and squat continuously. Read above shoulder level frequently. Strength up to 50 pounds occasionally up to 20 pounds frequently or negligible amounts continuously. Fine manipulation requirements of both hands are essential. Employee is exposed to bloodborne pathogens continuously and may be frequentlyoccasionally exposed to chemotherapeutics and toxic chemicals.
