Rate Caps for Submission:

Malpractice Must be included in rates.

Orientation half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours

On Call (Daily) xx3301xx3500 Per 24 hours to include 4 hours gratis

Holiday On Call (Daily) xx4951.50 to xx5250 Per 24 hours to include 4 hours gratis

Call Back xx376$425 Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.

Holiday Call Back xx564 to xx637.50 Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.

Average worked Callback Hours: 57h

Schedule/Availability Requirements:

10/7 am 10/12 am

10/31 am11/5 am

56 days in December

Shift type 24hour call for OB and Gyn No clinic or office patients.

Physicians do not need to remain in house but must be within 20minute response time.

Setting: Inpatient/Outpatient

Duties: Full scope OB GYN

FTE: 1.0

Required Procedures: Deliveries (Csection/vaginal) Gynecological surgeries

EMR: Cerner

Support Staff: Dedicated MA Practice Manager

Reason for Coverage: permanent provider decreasing coverage

Travel Preferences (see attachment for specific travel guidelines):

Preferred Lodging: none. In the event that the CommonSpirit Health preferred hotel is not available hotel/lodging rates will be reimbursed based on actual expense up to 1.5 times the current GSA rate for the market.

Air Travel: xx750/round trip cap

Rental Cars: xx65/day all in cap (including taxes and fees) regardless of car class.

Mileage: IRS Standard Rate

Please detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.
