Requirements for Social Worker Mental Health

• Bachelors Degree (BSW): Bachelor of Social Work (BSW) Psychology Sociology or a related fields.

• Licensed Clinical Social Worker (LCSW); Licensed Master Social Worker (LMSW; or Certified Social Worker (CSW)
• Must be US Citizen or Authorized to Work in the United States

Career Opportunity For Social Worker Mental Health Services
Your expertise in specializing in Mental Health will play a critical role in providing support counseling and interventions for individuals facing mental health challenges.

The Social Worker II serves as an integral member of the healthcare team providing assessments coordination treatment planning information and referral to community resources and other social work services to meet the complex needs of patients and families in the hospital and clinic settings. MSW will seek out supervision as needed with a Licensed Clinical Social Worker. Will use Patient Medical Record and HPF as primary information systems.

You will perform psychosocial assessments and develop a plan to meet the needs of patients with complex social issue/problems and arranges postdischarge services for patients as needed.

Under the general direction of the Social Work Manager/LCSW you will deliver ageappropriate social work care to patients and their caregivers in accordance with hospital policies and procedures and state and federal regulations.

Your responsibility will include but not limited to the following activities:

• Assist with adoptions abuse and neglect cases including making referrals to state and federal agencies

• Participate in the weekly Complex Case Review meeting and the daily patient care conferences

• Provide resources and make referrals to community agencies for ongoing counseling and or continued care

• Maintain an updated list of providers for postdischarge care

• Collaborative communication with postdischarge providers

• Update Case Managers on variance to postdischarge plans

• Monitor and provide reports to the DCM and hospital leadership as requested

• Maintain knowledge of regulatory requirements and community resources

• Other duties as assigned.
