We have a variety of schedules to support work life balance!
In this role you will:
- Develop long term meaningful relationships with the patients and their families
- Provide hightech nursing care to technologydependent patients in their homes
- Collaborate with the healthcare team to ensure comprehensive patient care
- Perform assessments administer medications and educate patients and families
- Utilize innovative technology to monitor and manage complex medical conditions
We are looking for someone who:
- Holds a current LPN license in Vermont
- Two or more years of directly related home/public health hospice and/or maternal/child health nursing experience
- Previous home care experience a plus
- Possesses strong clinical skills and a commitment to patientcentered care
- Demonstrates excellent communication collaboration and critical thinking skills
