Job Description

$20000 sign on bonus up to $20k in loan forgiveness and $2k/mo for 3 months housing stipend eligible for full time positions prorated for part time roles! Work life balance with flexibility and autonomy!

The Adult Home Health team delivers care to patients in the community providing a wide range of skilled nursing care. We are a comprehensive team comprised of RNs Physical Therapists LPNs LNAs SLPs Social Workers and administrative support all collaborating together to support patients to remain in living in the setting of their choice their home.

POSITION SUMMARY:


To provide a wide range of basic physical therapy services in a home care setting under close clinical supervision.


MINIMUM QUALIFICATION STANDARDS:


Bachelors degree or higher in physical therapy
Two years or more general PT experience preferred; or a year of experience at the PT I level at the University of Vermont Health Network Home Health & Hospice with a strong job performance record.

Home health experience preferred

Current Vermont Physical Therapist license
