To role model ESR Inc.s Core Values by providing Individuals in service the ability to reach their person centered plan/goals; supporting community integration while providing the necessary supports as described in the Coordinated Support Plan.
ESSENTIAL FUNCTIONS:
Professional Conduct: Comply with Basic Performance standards as identified in the Employee Guidelines and display behavior typically ascribed to a respected professional in human services including compliance with all applicable laws regulations policies and protocols and ESRs core values. Ability to establish and maintain positive and professional relationships with Individuals served and other customer groups.
Person Centered Support: Supports provided to individuals in our service should be delivered using a person centered approach following the active treatment plan to ensure that individuals are the primary guide for the selection structure and use of supports. Assist with daily care tasks such as feeding or toileting.
Facilitation of Services: Provide immediate supervision and quality control to individuals employed at the community crew work sites and the community program service hubs to assure the safe efficient and effective completion of tasks performed by individuals. Facilitate instructional active treatment and positive support strategies support individuals in communication and problem solving at the job site and observe and evaluate individuals activity to maximize productivity and integration. Flexibility to possible position adjustments is required.
Document: Accurately complete maintain and submit all documentation and written records as required by CARF and other licensing rules regulations and policies or procedures of ESR Inc.
Safety and Transport: Adhere to ESRs Safety guidelines including but not limited to the following. Properly administer medication as applicable according to the individual treatment plan. Operate vehicle in a safe and efficient manner observing all state laws route schedules and company policies. Perform routine inspections maintenance and documentation for all vehicles according to requirements established by ESR Inc. Report any repair or maintenance needs to the Program Supervisor or designee. Maintain structure and order on the vehicle. Respond to passenger emergencies and other incidents according to ESR Inc. guidelines policies and protocols. Report any unusual circumstances regarding passengers or vehicle operation to the program supervisor.
Training and Development: Participate in approved workshops formal educational classes inservice training and other professional development activities. Meet all explicit training requirements such as behavior intervention maltreatment of vulnerable adults lifting techniques etc. Maintain current certification according to agency schedule for CPR First Aid and Medication Administration.
MINIMUM QUALIFICATIONS:
A high school diploma or GED certification is required. Dependability and reliability is required. Position requires dependable transportation a current drivers license car insurance and a clear driving record with at least five year of driving experience. Must be able pass a comprehensive DHS background study and preemployment drug screen. Requires basic language written and computer skills that are necessary for communication and documentation in a timely manner. Must be able to work well with others and promote a positive work environment.
