A growing Healthcare Group is seeking a Licensed Practical Nurse (LPN) to work as a Care Coordinator Chronic Care Management (CCM) while working out of an office located in Brooklyn NY.

Responsibilities:

Review patients medical records to ensure that their plan of care is being followed and that patients medical management needs are being met.
Working collaboratively with behavioral coordinators PMDs and psychiatrists to resolve any gaps inpatients treatment plans.
Your responsibilities will include chart audits and reviews medical charting maintaining a patient registry and attending care team meetings.

Requirements

Requirements:

Valid LPN license in NY
Minimum 1 year of CCM experience
