YOUR CREDENTIALS

• Specialty certification: Physical Therapist (PT) License

• Statespecific license (if required)

• BLS ACLS DL SS Card

• COVID/Flu Vaccination (Exemptions allowed)

• Updated resume 3 references

Note: For Veteran Association (VA) roles a license from any state qualifies.
