Job Description
10 Points is urgently hiring excellent and experienced Social Worker for our school sites in Chicago IL for school year 202425. Interviews are being held. Please call (323)OR email for more information.

Job Responsibilities:
The following represents some major areas of concern in Social Worker.
- Develop and implement individualized care plans in collaboration with clients their families and other members of the interdisciplinary team.
- Provide counseling and support to individuals dealing with a range of issues such as mental health concerns substance abuse domestic violence and housing instability.
- Maintain accurate and uptodate records of client interactions progress and outcomes in compliance with organizational policies and regulatory requirements.
- Collaborate with other professionals such as medical professionals therapists and educators to ensure a holistic approach to client care.
Requirements
- Master s Degree in Social Work (MSW) from an accredited program.
- Current state licensure as a Licensed Social Worker (LSW) in Illinois.
- 2 years of experience in providing social work services.
- School clearly wants someone who has experience in School setting and is very well versed with IEPs 504 plans.
