Do you thrive in an intimate environment and have a passion for delivering compassionate care while developing meaningful and impactful relationships UVM Home Health & Hospice is seeking a skilled and dedicated High Tech LPN to join our growing team.

In this role you will be working 4 nights a week with a young adult client to provide specialized nursing care in their home and at their local summer camp. Experience is not required; we will thoroughly train in this specialty and provide ongoing support.

In this role you will:

• Develop long term meaningful relationships with the patients and their families
• Provide hightech nursing care to technologydependent patients in their homes
• Collaborate with the healthcare team to ensure comprehensive patient care
• Perform assessments administer medications and educate patients and families
• Utilize innovative technology to monitor and manage complex medical conditions
• Four 10hr shifts nights

We are looking for someone who:

• Holds a current LPN license in Vermont
• Two or more years of directly related home/public health hospice and/or maternal/child health nursing experience
• Previous home care experience a plus
• Possesses strong clinical skills and a commitment to patientcentered care
• Demonstrates excellent communication collaboration and critical thinking skills
