Pay: $35.25 $43.85 DOE Job Summary: We are seeking a dedicated and compassionate Primary Care Registered Nurse (RN) to join our healthcare team in New Hampshire. The ideal candidate will provide comprehensive nursing care to patients in a primary care setting collaborating with physicians and other healthcare professionals to promote optimal patient health and wellbeing.
Key Responsibilities:
- Conduct patient assessments including medical history and physical examinations.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Administer medications and treatments as prescribed by the physician.
- Monitor and document patient progress response to treatments and any changes in condition.
- Educate patients and their families on health management disease prevention and wellness strategies.
- Provide support and guidance to patients regarding lifestyle changes and selfcare practices.
- Assist with diagnostic tests and procedures as needed ensuring patient comfort and safety.
- Maintain accurate and timely documentation in electronic health records (EHR).
- Participate in quality improvement initiatives and adhere to hospital policies and procedures.
- Foster a positive and supportive environment for patients families and colleagues.
- Stay current with nursing practices advancements in healthcare and relevant regulations.
Qualifications:
- Current Registered Nurse (RN) license in New Hampshire.
- Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.
- Minimum of 3 years of experience in a primary care or outpatient setting preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a teamoriented environment.
- Proficient in using electronic health record (EHR) systems and other healthcare technologies.
- BLS and ACLS certification required.
