Job Description

At Ellenhorn we specialize in community integration programs designed to help individuals receiving psychiatric services maintain their social roles and stay connected to supportive networks. Utilizing the evidencebased Program for Assertive Community Treatment (PACT) model we provide an effective alternative to traditional care. Our multidisciplinary team including psychiatrists nurses primary case workers/clinicians and recovery coaches collaborates daily to offer comprehensive care. With low caseloads and communitybased work we ensure personalized and focused care for our clients. Established 18 years ago Ellenhorn has grown to employ 95 dedicated professionals who benefit from a supportive and collaborative work environment.

Role Overview:

As a Registered Nurse at Ellenhorn you will coordinate the medication and health needs of a caseload of 10 clients. This inperson role requires working five days per week and involves providing direct clinical services in various settings including the office clients homes and community environments. You will work closely with a dedicated team to ensure clients receive comprehensive and compassionate care.

Salary: $80000 $100000

Responsibilities:

  • Provide direct clinical services focusing on psychiatric medications and physical health to clients individually in various settings.

  • Continuously assess clients physical health symptoms and response to treatment making necessary changes to treatment plans.

  • Manage and administer medication in collaboration with the team psychiatrist including pharmacy coordination and insurance navigation.

  • Independently problemsolve issues related to medication and health.

  • Educate clients and families about medication and health.

  • Conduct holistic and comprehensive medical assessments with team members.

  • Advocate for clients rights and preferences providing education and support to their families.

  • Assume primary responsibility for case management developing implementing evaluating and revising treatment goals and plans in collaboration with clients and the team.

  • Liaise with community agencies and families to coordinate treatment attending medical appointments with clients as needed.

  • Participate in oncall rotation for crisis intervention/support 24/7.

  • Document client progress to maintain a permanent record of client activity according to established methods and procedures.

Skills and Qualifications:

  • Registered Nurse with a licence to practice in New York.

  • Minimum one year of psychiatric nursing experience.

  • Minimum one year of experience in a clinical psychiatric environment.

  • Excellent organizational communication and interpersonal skills.

  • Ability to prioritize tasks and manage competing priorities.

  • Dependability and responsiveness.

  • Competency in establishing supportive trusting relationships with clients with severe and persistent mental illnesses and their families.

Preferred Qualifications:

  • Experience working in a communitybased setting.

  • Familiarity with the PACT model.
