Role Overview: We are seeking an experienced home health Registered Nurse who is familiar with completing OASIS assessments. The ideal candidate should be a team player who is flexible willing to learn and teach capable of working independently and part of a team supervisory experience a plus! In this RN position with We Care Home Health you will play a crucial role in supporting our clinical staff admitting and caring for new clients and ensuring toplevel care is provided while guaranteeing their safety comfort and overall wellbeing.

Duties:

• Admit new clients to the agency and complete recertifications/discharges as necessary utilizing OASIS assessments.
• Complete routine client visits.
• Be part of the on call team on a rotating basis to share the Supervising RN responsibilities with other RNs and participate in the contingency plan for 24/7 clients.
• Perform other duties as discussed or assigned

Qualifications:

• Graduate of an accredited nursing program with a current license as a Registered Nurse in the state of Colorado
• Minimum of two (2) years nursing experience in home care and at least one (1) year of supervisory experience
• Demonstrated ability to supervise coordinate and direct clinical personnel
• Knowledge of Medicare/Medicaid and ACHC standards preferred
• BLS certified
• Excellent written and verbal communication skills and strong interpersonal skills including the ability to market and relate tactfully with clients and other members of the community
• Must be a licensed driver with an automobile that is insured in accordance with state and/or Agency requirements
