Currently we are partnered with a preferred client in Santa Monica CA in search of a highly talented Lead Care Manager LCSW.

Job Summary:
Lead Care Manager LCSW at our Health Center located in Santa Monica CA. The position available is for Full Time 12:00pm8:30pm shift.

Case Management is a collaborative practice that includes patients caregivers nurses social workers physicians payers support staff other practitioners and the community. The Case Management process facilitates communication and care coordination along a continuum through effective transitional care management. Recognizing the patients right to selfdetermination the significance of the social determinants of health and the complexities of care (barriers to coping abuse mental health substance use lack of resources psychosocial issues etc.).

Required qualifications:

• Masters Degree in Social Work from an accredited program.

• Upon hire: California Clinical Social Worker License
• 2 years of experience in healthcare related field (Acute Ambulatory or PostAcute)
• 2 years of experience in Acute Care Management (Care Coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers.

Screening Questions:
Do you have a Masters Degree in Social Work
Do you have 2 years of acute care inpatient experience specific to utilization review care coordination & discharge planning
Do you have experience providing supervision hours in an acute care hospital setting
This position requires a California Clinical Social Worker License upon hire as required in our job posting can you meet this requirement
