Job Description
This is a fulltime position based in Palm Springs CA.
The individual in this position performs psychosocial assessments and develops a plan to meet the needs of patients with complex social issue/problems and arranges postdischarge services for patients as needed.
Assist with adoptions abuse and neglect cases including making referrals to state and federal agencies
Participate in the weekly Complex Case Review meeting and the daily patient care conferences
Provide resources and make referrals to community agencies for ongoing counseling and or continued care
Maintain an updated list of providers for postdischarge care e) collaborative communication with postdischarge providers
Update Case Managers on variance to postdischarge plans
Monitor and provide reports to the DCM and hospital leadership as requested
Maintain knowledge of regulatory requirements and community resources
Other duties as assigned. Under general direction of the Social Work Manager/LCSW delivers ageappropriate social work care to patients and their caregivers in accordance with hospital policies and procedures and state and federal regulations.
Requirements
At least one year of social work experience.
