Physical Therapist

Overview

As a Physical Therapist on our clients Home Health team you will be responsible for providing Physical Therapy services to patients in their homes. Your role will involve assessing patient functioning levels developing treatment programs to enhance independence and physical/cognitive functioning and educating patients and caregivers. You will also supervise physical therapy assistants and when appropriate certified nursing assistants and report on patient progress.

Key Responsibilities

Assessments & Treatment Programs: Perform Physical Therapy assessments and develop individualized treatment programs.

Reporting: Record and report patients reactions to treatments and any changes in their condition.

Education: Instruct patients and caregivers on therapy programs and the use of therapeutic devices.

Supervision: Supervise physical therapy assistants and when appropriate certified nursing assistants.

Documentation: Prepare reports and participate in staff and Agency development activities.

Schedule and Shift Details

Work Schedule: MondayFriday 8:00 AM/9:00 AM to 5:00 PM.

Weekend Shift: 1 weekend per month (Saturday & Sunday; day shift with scheduled patients).

Holiday Coverage: Required to work 2 holidays per year (1 summer 1 winter).

OnCall: No oncall duties.

Travel

Local Travel: This position requires travel to residential homes within the local area.

Qualifications

Required:

Education: Masters degree or higher in Physical Therapy.

Experience: Minimum of 1 year in a healthcare setting.

Licensure: Valid Physical Therapist license in the state of Illinois (IL).

Drivers License: Valid regular drivers license (DL).

Residential Home Health & Hospice Application History: Must not have applied to or worked for Residential Home Health & Hospice in the past 6 months.
