Job Description

3 Months contract with a Local Authority

Job Summary:

Liverpool Early Help utilises a strength based approach to provide empowering interventions to children and families to help address emerging worries and affect positive change to improve the longer term outcomes of children and young people.

As the lead professional you will coordinate support with our multiagency partners to ensure that a whole family approach is taken to address the identified needs of the family.

We are looking to recruit an Outreach family support worker to undertake early help assessments with families create support plans and chair team around the family meetings to review the progress and identify any additional areas of support.

The role involves direct work with children and young people to explore their views and worries to ensure that the plans/interventions are helping to improve their outcomes..

Key Duties/Accountabilities

Taking whole case responsibility to provide high quality outreach and homebased Early Help support to children and families using a whole family approach (level 2 and 3).

Delivering targeted support to children across age ranges (0 25*) and their families in order to prevent the need for statutory services and to ensure families access Universal services.

Between ages 1925 relates only to Adults where the SEND Code of Practice is applicable.

Act as case holders for complex cases that step down from childrens social care (level 3/2).

This will involve completing early help assessments arranging Team Around the Family meetings and supporting families to improve outcomes for their children by taking on the role of the Lead Professional and associated tasks.

Offer support on cases that become more complex and reach level 4. Support is continued on these cases to allow a smooth transition between early help and social care.

Provide intensive and targeted support to vulnerable children and young people. This includes bespoke packages and programmes. This also includes supporting adolescents with complex emotional and mental health problems.

Essential Experience Required:

Experience of direct work with children young people and their families is essential.

Experience of effective collaborative and multiagency working to support children and families is essential.

Experience of delivering outreach and home based support to children and families to address areas of identified need is essential.

Essential Qualification Required:

NVQ Level 3 in childcare and education NND Health and Social Care qualification is essential.

Additional information to note:

Working hours: 35 hours per week.

The role requires an Enhanced DBS check.
