Care Manager RN

Job Title: Care Manager (RN)
Work Location: Remote
Pay rate: $43.00/Hr. On W2.
Duration: 3 Months
Shift (Start/End Time): 8am5pm PST
Potential to Extend: Yes

DaytoDay Responsibilities:

• Outreaching members to enroll them in the Care Management (CM) program.
• Confirming eligibility for CM services.
• Building working care plans conducting medication reviews completing assessments and addressing care gaps.
• Documentation of each case which can take around 2 hours per case to complete.

Performance Expectations:

• Make 20 calls per day.
• Achieve 10 enrollments per week.

Required Skills/Experience:

• Time management skills to work in a fastpaced environment.
• Ability to work independently with excellent technical and documentation skills.
• Recent telephonic CM remote work experience with Managed Care and/or Medicare.

Preferred Skills/Experience:

• Strong technical skills.
• Knowledge of TruCare documentation system.
• Bilingual abilities (Spanish/Chinese/English) are strongly preferred.

Education Requirement:

• Associates degree

Software Skills Required:

• Proficiency in Microsoft Office Suites

Work Hours:

• 8 AM to 5 PM PST

Licensure:

• Must have a valid CA RN license
