About the Role: Our client is seeking a Case Manager to support physicians primary medical homes and interdisciplinary teams. The goal is to facilitate patient care enhance clinical outcomes and improve patient satisfaction while managing care costs. This role integrates resource utilization management care facilitation and discharge planning functions. The Case Manager also drives performance improvements by identifying areas needing enhancement.

Position Summary: As a Case Manager you will coordinate patient care progression ensuring effective and timely care. You will collaborate with physicians nurses and other healthcare team members to resolve system issues and discharge obstacles. The role involves monitoring patient progress facilitating necessary interventions and ensuring all care plan elements are communicated and documented.

Key Responsibilities:

  • Patient Care Facilitation: Coordinate and facilitate patient care across the continuum including discharge planning and resource utilization management.
  • Communication: Maintain active communication with the healthcare team to provide timely patient care.
  • Problem Resolution: Address and resolve system problems impeding diagnostic or treatment progress.
  • Data Management: Use data to drive decisions and plan performance improvement strategies.
  • Patient Advocacy: Serve as an advocate for the patients physical and emotional wellbeing.
  • Documentation: Complete all required documentation and communicate relevant information to the care team.

MustHaves:

  1. Education: Graduate of an accredited school of professional nursing or an accredited Master of Social Work program.
  2. Licensing: Current and valid license to practice as a Registered Nurse or Master Social Worker (LMSW) in the state of Texas.
  3. Certification: Certification in Case Management required within two (2) years of hire.
  4. Experience: Recent acute care case manager hospital experience. Three (3) years of nursing or social work experience in an acute hospitalbased setting preferred or three (3) years of experience in a comparable clinical setting.

NiceToHaves:

  1. Experience: Experience in utilization management case management discharge planning or other cost/quality management programs.
  2. Skills: Excellent interpersonal communication negotiation and leadership skills.
  3. Knowledge: Understanding of discharge planning utilization management performance improvement disease or population management and managed care reimbursement.

Minimum and Preferred Qualifications:

  • Education: Graduate of an accredited school of professional nursing or Master of Social Work program. Bachelors of Nursing (BSN) or Masters degree preferred.
  • Licenses/Certifications: Current and valid license to practice as a Registered Nurse or Master Social Worker (LMSW) in Texas. CCM preferred.
