JOB SUMMARY
The Physical Therapist is a licensed professional who directs, supervises, evaluates and provides physical therapy services to the patients as prescribed by the attending physician. May supervise Physical Therapist Assistants and Aides.
QUALIFICATIONS
Graduate from an accredited physical therapy school. Preferably two years of clinical experience (if applicable). Capable of lifting the equivalent weight of an adult-size patient. Licensed by the state in which the Physical Therapist resides or practices Physical Therapy according to state requirements.
PHYSICAL DEMANDS
Requires moderately heavy physical exertion on a regular and reoccurring basis such as extensive driving, assisting patients in transfer activities (wheelchair to bed, to tub, to commodes) and providing substantial support to individuals in ambulation therapy. Regularly required to use hands and arms. Frequently required to stand, walk, reach with hands and arms, stoop, kneel, crouch, or crawl. Occasionally required to sit. Occasionally lift and/or move over 100 pounds. Specific vision abilities include close, peripheral, depth perception and ability to adjust focus.
WORKING CONDITIONS
General office, health care setting - exposure to infectious disease.
ESSENTIAL FUNCTIONS
1. Provides treatment as ordered by the attending physician. Interprets, plans, establishes, and exercises proper treatment program in accordance with the physician s referral, periodically, reviewing the patient s program and response and making recommendations to the physician relative to the need of continuing or discontinuing the physical therapy treatment. Assists the physician in evaluating disability, injury, or disease process of individual patients and in evaluating the patient s level of function.
2. Contacts the physician directly for patient reports and care as needed. Observes, records, and reports to the physician the patient s reaction to the therapy program or any changes in the patient s condition. Submits to the physician and the Program Supervisor appropriate written reports to the patient s progress at the designated intervals using appropriate and designated forms.
3. Assist in determining the need of the patients an in developing the total plan of care.
4. Instructs, teaches and supervises patients regarding physical therapy procedures as related to the patient s therapy programs.
5. May supervise Physical Therapist Assistant(s) and co-sign Physical Therapists Assistant s progress notes.
6. Adheres to all applicable federal, state, local and company maintained standards of care and ethics and policies concerned with the practice of physical therapy and among other things, personnel qualifications.
7. Participates in staff development activities and in in-services education programs.
8. Demonstrates evidence of continuing education conducted for in-service sessions.
9. Meets productivity and utilization expectations as established by the Company and the facility.
10. Completes timely clinical documentation to include functional goal-setting, progress and therapeutic approach.
11. Completes administrative/billing documentation timely and accurately.
12. Covers more than one facility as required by the need of the Company.
