Inventory Control Assistant

1st Care Home Health Services, FL is seeking a Full time Physical Therapist to be part of the team caring for our Patients at home.

Job Responsibilities

• PT provides a comprehensive patient evaluation at the initial visit upon receipt of referral. Provides therapeutic interventions in accordance with the physician-approved plan of care.
  
• Reassesses the patient at a minimum of every 30 days. Makes necessary revisions to the patient s plan of care in conjunction with the physician.
  
• Completes OASIS assessments timely, accurately, and according to OASIS user s manual definitions.
  
• Provides timely documentation for each therapy visit according to required Medicare/Medicaid/Insurance guidelines.
  
• Provides coordination of care by participating with an interdisciplinary team and by observing, documenting, and reporting the patient s response to treatment and any change in the patient s condition to the team.
  
• Identifies the patient s need for other disciplines, services, and/or medical equipment and initiates the appropriate referrals. Provides resources for future needs.
  
• Maintains confidentiality of information relating to the physical therapist-client relationship.
  
• Supervises the PTA and Home Health Aide when applicable and communicates plan of care changes to the Aide and other appropriate staff as necessary.
  
• Performs other duties as assigned.
  

Requirements

• Active Physical Therapist license for the state of FLORIDA
  
• A minimum of one year of PT experience is required; home health is preferred
  
• Able to travel the assigned territory based on geographical area offered
  
• Active CPR Certification
  
• Active drivers license and reliable transportation
  
• Electronic documentation experience
