Job Description

1 month contract with A Local Authority

Job Summary:

Assist residents in securing their next stage housing providing resettlement support so that they successfully move into and sustain their new home.

To provide appropriate floating support for 6 months and to ensure they are linked into necessary support services thereafter.

To provide assertive client centred keyworking risk assessment and support planning to a caseload of approximately 10 clients aimed at supporting them to maintain their current accommodation and link into services and meaningful activities which help them to improve their physical and mental wellbeing wellbeing and life skills.

To ensure continual highquality assessment of risk to residents and colleagues and ensure abidance to all council policies and procedures.

Key Duties/Accountabilities:

Provide a highquality customer focussed service with clients being treated with courtesy and respect and meet all council standards and targets relating to communication with residents.

Advocate for residents where required providing practical support if needed to ensure that their rooms are habitable and safe and where necessary to accompany residents to appointments. e.g. Health Services DWP viewings.

Continually monitor and safeguard residents physical and mental health and liaise closely with appropriate services e.g. ASC CMHT Health Services Criminal Justice etc and working collaboratively with other services by referring and reporting to ASC SOVA MARAC panels and other services involved in your resident s care.

Arrange bookings for new residents coordinating interviewing and assessing risk and carry out Risk Management and SOVA assessments for all new residents and inducting them to the Health and Safety requirements of the accommodation.

Deal proactively with behavioural issues within the hostel managing risk safely and appropriately and where necessary represent the council in court in conjunction with specialist agencies and coordinate and carry out evictions relating to antisocial behaviour and other breaches of licence.

Essential Experience Required:

Experience of working with homeless people with complex support needs.

Experience of recovery focused and harm minimisation service provision for clients with multiple and complex needs.

Experience of risk assessment keyworking and support planning.

Experience of case coordination in a multiagency setting.

Essential Qualifications Required:

Knowledge of the impact of Adverse Child Experiences and the principles of Trauma Informed Care when dealing with homeless people who have complex needs.

Additional information to note:

The post holder will be required to undertake a Criminal Records Bureau check on appointment and every three years thereafter whilst in post.
