Personal Health Record - RN

Responsibilities:

• Primary duty conduct longterm care screenings in clients homes; may work with social workers from local Department of Social Services.
• May be required to assist with other PHN clinical duties.

Requirements:

• Ability to assess functional medical or nursing and social needs of adults and children applying for Medicaid longterm care services.
• Ability to work independently and apply sound nursing judgment.
• Ability to work with a diverse population Assures documentation of screening is accurate and complete.
• Ability to work with computer systems including email Microsoft applications and patient database systems.
• Maintains legal parameters of patient confidentiality Completes required training within established timeframes.
• Adheres to VDH Driver Safety Policy; maintains a valid drivers license and the ability to travel throughout the district.
• Experience in longterm care or public health helpful
• Licensed Registered Nurse required CPR certification required.
