NDIS Registered Provider is seeking an experienced Specialist Support Coordinator to join their boutique Support Coordinator team.
You will have at least 2 years experience working as a Specialist Support Coordinator with relevant degree level qualifications e.g. social work and the ability to support clients with complex support needs receiving services from large multidisciplinary teams.
The role is located at modern offices in Mulgrave and is full time Monday to Friday 9am to 5.00pm or part time (hours/days to be agreed).
You must have excellent verbal and written communication skills and the ability to quickly engage and gain trust with a diverse group of people. Attention to detail good time management skills and ability to work to deadlines are essential.
The role will support both Specialist Support Coordination and Level 2 Support Coordination clients as required by the business.
Requirements
- Mulgrave office based full time or part time working as part of a small team.
- Understand each clients NDIS plan and goals align the coordination of supports and multidisciplinary teams and build capacity of clients.
- Ensure reporting requirements are completed as per NDIS guidelines.
- Answer all enquiries from clients and stakeholders in a timely manner.
- Manage and maintain client records and information proficiently via Brevity Care.
- Triage new client referrals when required and seek alternative service providers if the company is not the best fit.
- Mentor other team members to grow and develop in their roles (everyone brings their own skills and experience to the table).
- Use existing contacts and networks to grow the Support Coordination teams client portfolio.
- 2 years experience in a Specialist Support Coordination NDIS role.
- Degree level qualification in a relevant field social worker psychology allied health community services or mental health.
- Experience of working with and coordinating support for people with complex disabilities high intensity care including mental health and psychosocial disabilities autism & ADHD housing or leaving school is also highly regarded.
- Ability to undertake research deliver information in an easytounderstand format and build key relationships with a range of diverse stakeholders is critical.
- Must be highly organised and be able to manage competing priorities whilst delivering consistently highquality outcomes.
- Full competence with computer systems apps and other technology relevant to the role
