Tasks

Support hospital discharges by coordinating with healthcare providers and
community resources.
Conduct assessments and develop care plans that promote independence and
wellbeing.
Work collaboratively with individuals their families and multidisciplinary
teams to ensure smooth transitions from hospital to home.
Apply a strengthsbased approach to support individuals in achieving their
personal goals.
Navigate complex cases including those involving the Court of Protection
ensuring legal and ethical standards are met.
Travel within the community to provide support and conduct assessments as
needed

Requirements

Qualified Social Worker with current registration.
Proven experience in the hospital discharge process.
Experience or knowledge of working with complex cases and the Court of
Protection.
A valid drivers license and access to a vehicle for travel within the community
