Key Responsibilities

• Provide management and delivery of patient care plans
  

• Support patients in Springfield and surrounding areas
  

• Reward the accomplishment of key achievements during the first year
  
  

Schedule and Shift Details

• It is 7 evenings in a row from 5 PM to 8 AM Mon to Sun after hours on call and making visits if needed in the Springfield IL area. Then would be off 7 days in a row.
  
  

Requirements

• RN license in the state you work
  

• Associate degree or higher from an accredited School of Nursing
  

• Two years of RN experience hospice experience preferred
  

• Current driver s license and ability to spend 20% of your day driving to/from patient locations
  

• 1 years of experience in ANY of the following:
  

• Electronic Medical Record (EMR)
  

• Other EMR
  

• TruChart EHR
