Job Purpose: We are seeking a dedicated Social Worker with a minimum of one year s postqualification experience to join our 1625 Transition Team. The role involves managing a complex workload working within relevant legislation and Peterborough City Councils procedures and collaborating with young people and their families to assess needs and deliver services.

Main Duties and Responsibilities:

• Build effective relationships with young people and their families to assess needs and promote participation in decisionmaking.
  
• Conduct assessments and manage risks for disabled children and young people.
  
• Plan implement and review interventions for disabled young people and their families.
  
• Independently manage workload seeking support when necessary.
  
• Utilize supervision for effective practice and career development.
  
• Maintain accurate and confidential records producing clear and structured reports.
  
• Collaborate with colleagues and organizations to support disabled young people and their families promoting positive change and independence.
  
• Uphold the Council s Equal Opportunities Policy challenging or reporting discriminatory behaviour.
  
• Meet the Professional Capabilities Framework and HCPC registration requirements.
