Job Purpose: We are seeking a dedicated Social Worker with a minimum of one year s postqualification experience to join our 1625 Transition Team. The role involves managing a complex workload working within relevant legislation and Peterborough City Councils procedures and collaborating with young people and their families to assess needs and deliver services.
Main Duties and Responsibilities:
- Build effective relationships with young people and their families to assess needs and promote participation in decisionmaking.
- Conduct assessments and manage risks for disabled children and young people.
- Plan implement and review interventions for disabled young people and their families.
- Independently manage workload seeking support when necessary.
- Utilize supervision for effective practice and career development.
- Maintain accurate and confidential records producing clear and structured reports.
- Collaborate with colleagues and organizations to support disabled young people and their families promoting positive change and independence.
- Uphold the Council s Equal Opportunities Policy challenging or reporting discriminatory behaviour.
- Meet the Professional Capabilities Framework and HCPC registration requirements.
Requirements
Qualified Social Worker with HCPC registration.
Minimum one year s postqualification experience.
Strong assessment planning and intervention skills.
Excellent recordkeeping and reportwriting abilities.
Proactive collaborative and able to manage a complex workload.
