Job Description

We are a leading healthcare institution committed to providing exceptional pharmaceutical care to our community. We are currently seeking a dedicated and experienced Pharmacy Clinical Coordinator to join our team. This position offers the opportunity to work in a dynamic and collaborative environment focused on patientcentered care.

Responsibilities:

Serve as a key member of the pharmacy team collaborating with pharmacists physicians and other healthcare professionals to optimize patient care.
Develop implement and evaluate clinical pharmacy programs and services to enhance patient outcomes.
Provide direct patient care through medication therapy management medication reconciliation and patient education initiatives.
Conduct comprehensive medication reviews and assessments to identify drugrelated problems and recommend appropriate interventions.
Participate in medication safety initiatives adverse drug event monitoring and quality improvement activities.
Serve as a resource for healthcare providers regarding drug information dosing drug interactions and pharmacotherapy guidelines.
Maintain accurate documentation of clinical interventions patient consultations and outcomes.
Stay current with advances in pharmacy practice pharmacotherapy and healthcare regulations.
Assist with staff education and training on clinical pharmacy topics.

Qualifications:

Current licensure as a Pharmacist in the state of West Virginia.
Minimum of 5 years of clinical pharmacy experience in a hospital or healthcare setting preferred.
Strong clinical knowledge and understanding of evidencebased pharmacotherapy principles.
Excellent communication interpersonal and organizational skills.
Ability to work independently as well as collaboratively within a multidisciplinary team.
Proficiency in pharmacy software systems and electronic health records.
Commitment to continuous learning and professional development.

Requirements

  • Bachelor of Science degree in Pharmacy from an accredited college of pharmacy
  • Current Pharmacist license to practice in the state required on hire
  • One (1) year of previous hospital pharmacy experience


- Collect specimens for laboratory examination through venipuncture, various culture techniques, etc. - Adhere to OSHA, CLIA, and Joint Commission regulations to ensure compliance and quality assurance. - Work independently without supervision or assistance, demonstrating proficiency and reliability. - Respond promptly to codes, traumas, and emergencies, providing immediate support as needed. - Operate and maintain laboratory instruments, ensuring proper functionality and accuracy of test results. - Conduct quality control procedures, maintaining documentation of all activities. - Keep accurate logs and records, including data entry and retrieval of patient results, as well as accession of patient specimens. - Report STAT results within 60 minutes, perform manual tests as required, and set up bacteria on appropriate media as needed.
